Patient Services
The following services can be provided in the patient’s home, an assisted living facility or nursing home facility.
- Regular visits by a registered nurse to monitor the patient’s condition and provide appropriate care and comfort measures
- 24 hour availability of hospice nurses for phone consultation and/or emergency visits
- Home health aide visits for assistance with personal care and activities of daily living
- Volunteer support for friendly visits and assistance to the caregiver with errands or other help requested by the family
- Medications related to the terminal illness for symptom control and pain relief
- Physical Therapy for symptom control
- Massage Therapy for comfort
- Chaplain services for spiritual care and support
- Social Work services for coordination of community resources and guidance and support to the patient and loved one
- Medical equipment and supplies to ensure comfort, safety and quality care
